About The Position

Join our team as a Memory Care and Programming Manager to oversee the management and performance of the Memory Care neighborhood as well as the Programming Department of Continuing Care (skilled nursing, long-term care, memory care, and assisted living). This position is responsible for meeting the programming needs of all the CC residents, in addition to ensuring the highest quality of resident care and customer service in Memory Care. Requirements

  • Qualified as an Activity Director or CTRS, in accordance with state regulations.
  • Minimum of 3 years of experience in LTC or AL leadership required. Education will be considered in lieu of experience.
  • Experience with the elderly and understanding of the dementia process and its impact on daily living and activity programming.
  • Must have knowledge of the geriatric population and the aging process, including the physical, psychological and social needs of the elderly.
  • Meets state-specific requirements for management certification of assisted living and/or personal care facilities (within 90 days of hire)
  • National Certification Council for Activity Professionals (ADCP or ADC) as required by the State

Nice To Haves

  • ACC (Activity Consultant Certified), CDP (Certified Dementia Practitioner), or CTRS (Certified Therapeutic Recreation Specialist) is preferred.

Responsibilities

  • Create and maintain a strong, cross-functional team philosophy by encouraging communication, relationship building, dedication to customer service, and the commitment to quality resident care.
  • Conduct pre-residency assessments and screening of potential residents make recommendations for the appropriate level of residency in conjunction with the Admissions Committee and other team members.
  • Evaluate the psychosocial and activity of daily living (ADL) needs of residents in Memory Care and coordinate care and services as needed.
  • Lead the service/care plan process by completing the following: partnering with other team members to develop a service/care plan based on the resident’s psychosocial needs and abilities; validating and ensuring each resident’s plan of care is being followed by all members of the neighborhood; maintaining a care conference schedule and coordinating the interdisciplinary care/service plan meetings with family members
  • Assess residents’ needs and abilities on an ongoing basis for program planning, assessment completion, and MDS.
  • Interacts directly with residents and family when able to gather information for assessments, MDS, and care plans
  • Provide oversight of the delivery of customized, individualized resident services.
  • Create daily, weekly, and special event schedules to be posted in the neighborhood and made available to residents and their families as requested.
  • Ensure compliance with Federal, State, and County requirements related to activity program documentation, including schedules, MDS, and Care Plans.
  • Create partnerships with all departments to stimulate and promote a social and home-like atmosphere resulting in a warm and inspirational environment for residents which creates community, celebrates life, and allows them to share their gifts
