About The Position

We are looking for a high energy leader to support our East Valley area as a Member Care Supervisor! At Home Assist Health, the Member Care Supervisor HCBS is instrumental in providing quality, independent lifestyles for people living with disabilities, and ensuring that they have the best care experience possible. The Member Care Supervisor HCBS is dedicated to the success of our Direct Support Professionals (DSP), Direct Care Workers (DCW) and Member Care Coordinators (MCC); providing them an employment experience that is positive and rewarding. The Member Care Supervisor HCBS supports the Associate Director, HCBS to ensure smooth running of daily operations for the department. Member Care Supervisors work in a team environment to create and supervise member care plans/programs, schedule and guarantee caregiving services such as personal care assistance, attendant care, respite, and habilitation. They work with payer representatives (i.e. case management, support coordinator, etc.) to ensure member needs are being met and maintained on an ongoing basis. Member Care Supervisors maintain a keen focus on ensuring quality care for HAH members. Requirements

  • High school diploma or GED required. Bachelor’s degree preferred.
  • AHCCCS Direct Care Worker training or current AZBN CNA/LNA certification.
  • AHCCCS Direct Care Worker 2 – DDD training.
  • Article 9 Training
  • Prevention & Support Training
  • Habilitation Skills / ISP Process Training
  • Requires current CPR & First Aid certification.
  • Ability to read, write, and speak English effectively, bi-lingual in Spanish preferred.
  • Excellent communication, time management, and organizational skills.
  • Excellent customer service skills.
  • Proficiency with office software such as Word, Excel, PowerPoint, and Access.
  • Must possess strong interviewing, counseling, leadership, management, planning and organizational skills.
  • Requires a basic understanding and knowledge of compliance and safety issues in non-medical home care.
  • Requires use of personal automobile for making home visits.
  • 2 years’ Direct Care experience in DD/Habilitation
  • Minimum 1 year experience in service coordination, case management and/or related work experience.
  • Supervisory experience.
  • Must have DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
  • Must pass Criminal Background if applying for DPS Level 1 Fingerprint Clearance Card.
  • Must pass APS Registry Check.
  • AZ DES Central Registry Clearance
  • Must sign notarized Criminal History Affidavit at new hire and then annually
  • Must have a valid AZ driver’s license that is in good standing and maintain current vehicle insurance and registration
  • Must have proof of TB Clearance

Responsibilities

  • Being a positive representative of Home Assist Health upholding the company’s Mission, Vision and Purpose.
  • Assisting as needed in the professional training and development of Member Care Coordinators
  • Performing as the Program Manager when the Program Manager is absent or otherwise unavailable.
  • Supervising and intervening as needed to ensure that DSP and DCW provision of services are in compliance with service requirements, the individual member care plan, the habilitation program, and the payer expectations.
  • Developing habilitation programs for members authorized with habilitation services, supervising DSP administration of the habilitation program, and identifying opportunities for program improvement based on member progress, challenges, etc.
  • Staffing and scheduling of services in accordance to the service/care plan and authorizations.
  • Supervising, training, and mentoring a team of DSPs and DCWs.
  • Guaranteeing services by filling service gaps promptly and when needed will perform as back up DSP/DCW.
  • Promoting a positive care experience for our members by matching them with the DSP/DCW that best suits their needs.
  • Collecting, reviewing, and ensuring proper completion of EVV, program data reporting, and other reports/documentation as required.
  • Acting as the primary contact for payer representative (i.e. Case Manager/Support Coordinator/Relative) communicating regularly with regard to member care including but not limited to: authorizations, incidents, and changes in behavior/condition/needs/etc.
  • Initiating and participating as needed in the incident reporting process.
  • Immediately reporting any suspected or witnessed abuse, neglect, or any other potentially harmful and/or illegal activity.
  • Providing after-hours support to the Staffing Support Specialist on a rotating basis
  • Participating in program weekly huddles and completing case audits as they are due.
  • Conducting initial case assessments and new member intake (5 Day 1) as approved and overseen by supervisor.
  • Assist with other duties as assigned.
