The Member Care Scheduler will serve Home Care and Community Supported Living Members that are frail elders, persons with physical disabilities and/or persons with intellectual/developmental disabilities. The goal for you and your Team is to ensure that these Members receive the services that are authorized for them, which promote their overall well-being. Also, the goal for you and your Team is to ensure that Clarity Care’s promise to provide high quality care is being met. Understand the Members, their Assessment, their Needs and Outcomes. Utilize and maintain the Scheduling Software Program leading to appropriate Member Care, accurate schedules, increased internal and external customer satisfaction and efficient billing practices. Develop and maintain Home Care and Community Supported Living schedules to meet Member’s needs on a bi-weekly rotation. Schedules must match authorized services. Members are to receive services; Members are not to be left unattended. The Member Care Scheduler is heavily relied upon to ensure Clients receive services. Ability to multi-task and ability to work in a fast paced environment with excellent internal and external customer service skills. Answer phones and text messages with professionalism and respond appropriately to the needs of our internal and external customers. Assist with maintaining and updating employee exceptions on a daily basis. As Member conditions change, maintain and update all systems, processes, computer applications, and documentation, including Schedules. Such changes may require a change to the caregiver/employee Information/Schedules as well. Listen and respond to internal and external customers’ needs and concerns in a professional manner. Work with the Team to politely resolve such. Develop and maintain on-call schedule for direct care employees, must meet on-call expectations. On call responsibilities for evenings, weekends, and holidays on an established rotation schedule.

Requirements

  • A Bachelor’s degree in Human Services, Psychology or Supply Chain Management. An Associate’s Degree will be considered. Or, related field with two (2) years’ experience working with elders or persons with disabilities.
  • Possess advanced computer and telecommunication skills, with proficiency in Microsoft Office Suite.
  • Excellent organizational, time management, and critical thinking skills.
  • An energetic, outgoing, detail-oriented person who demonstrates strong written and verbal communication skills.
  • Ability to be resourceful and proactive when issues arise.
  • Must possess a valid WI driver’s license, and ability to meet agency driving standards.
  • Must provide proof of auto insurance.
  • Must pass periodic WI Caregiver Background Checks including Department of Justice Criminal Record Checks, and/or that of previous states of residence
