About The Position

The Medication Therapy Management (MTM) Pharmacist improves patient outcomes by conducting comprehensive medication reviews, providing individualized education, and identifying, preventing, and resolving medication-related problems. This role collaborates closely with physicians, care teams, and patients to optimize therapy, support shared clinical goals, and enhance medication safety and adherence across the health system. The MTM Pharmacist may also provide oversight of pharmacy staff in the absence of leadership and participate in projects and initiatives directed by the Pharmacy Leadership Team. Requirements

  • Doctor of Pharmacy (Pharm.D.) or active Iowa Board of Pharmacy waiver to provide pharmaceutical care as an RPh.
  • Must maintain current license to practice pharmacy in Iowa with the Iowa Board of Pharmacy Examiners to continue employment.
  • Valid driver’s license when driving any vehicle for work-related reasons.

Responsibilities

  • Review, verify, and dispense prescriptions accurately in compliance with state and federal regulations.
  • Assess prescriptions for appropriateness, dosing, interactions, and patient-specific factors; communicate with prescribers as needed.
  • Provide medication counseling and patient education to promote safe use, adherence, and positive health outcomes.
  • Administer immunizations and provide point-of-care clinical services as applicable.
  • Deliver excellent customer service by responding to patient questions and resolving medication-related concerns.
  • Supervise and support pharmacy technicians to ensure efficient workflow and accuracy.
  • Maintain accurate documentation and patient records in accordance with policy and regulatory requirements.
  • Participate in quality, safety, and inventory management activities to support pharmacy operations.
  • Maintain a clean, organized, and secure pharmacy environment.
  • Demonstrate professionalism, teamwork, and a commitment to continuous improvement and patient-centered care.
