Medication Management Pharmacist – Anticoagulation / GLP-1 / Emerging Disease States Location: Twin Falls, Idaho Organization: St. Luke’s Health System Schedule: Monday–Friday, 8:00 a.m. – 5:00 p.m. Position Summary The Medication Management Pharmacist is responsible for the delivery of high-quality pharmaceutical care to patients and customers of St. Luke’s Health System. This role will primarily support medication management services in anticoagulation and GLP-1 therapy, with opportunities to expand into additional disease states as services evolve. The pharmacist works collaboratively with providers, nurses, and other healthcare professionals to optimize medication therapy outcomes in accordance with medical center standards.
