About The Position

This is a full time day shift Medication Management Pharmacist position for the refill department. This position is based out of the Integrated Service Center in Plainfield, IN. This is a Monday - Friday hybrid position with a required 2 days onsite. 

Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes. In this role, the pharmacist will be responsible for conducting comprehensive chart reviews to identify and resolve medication‑related issues. Key responsibilities include evaluating potential drug interactions, duplicate therapies, vital signs/laboratory values, and addressing gaps in care related to medication management.

Prior experience in ambulatory care, population health or centralized pharmacy services is preferred, along with familiarity with protocol-driven care.

Requirements

• Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
• Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
• Requires 0-3 years of relevant experience.
• Requires current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist.
• Board certification or other specialized training may be required.
• Basic Life Support certification through the American Heart Association (AHA) may be required.
• May require travel to IU Health facilities.
• May require weekend rotations.

Nice To Haves
