This job assists providers with the refill and infusion processes by ensuring patient’s compliance with medication, lab monitoring, adherent to therapy by providing appropriate refill amounts on medication refill requests, maintaining accurate pharmacy preferences, identify discrepancies or problems with medication history information. This role communicates with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain and accurate and complete medication history for patients while also documenting the encounters within the electronic health record (EHR). Any problems identified are forwarded to a pharmacist for resolution. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Requirements

  • Education Required – High School diploma or equivalent
  • Work Experience Required – 2 years of experience as a Pharmacy Technician or equivalent health care experience with medications
  • Must have computer skills and dexterity required for data entry and retrieval of patient information.
  • Must have working knowledge of pharmaceutical mathematics.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
  • Must be able to operate standard pharmacy and office equipment.
  • Strong communication and interpersonal skills.

Nice To Haves

  • Preferred – 1 year of college or completion of a para-professional pharmacy training program
  • Preferred – Prior experience taking patient medication histories
  • Experience working with Healthfinch software
  • Preferred - Certified Pharmacy Technician License in the State of Practice or National Certification

Responsibilities

  • Accurately obtains and clarifies patients' home/chronic medication list by contacting community pharmacies, physician offices and/or extended care facilities, interviewing patient and/or family members to ensure proper identification of doses and drugs including prescription medications, over the counter drugs and alternative medications in a timely manner.
  • Utilizes the EHR to input patients’ home/chronic medication lists for the purposes of medication reconciliation under supervision of a physician.
  • Communicates discrepancies identified via the medication history process with the pharmacist and makes clarifications with the patient and prescribers as needed
  • Accurately collects patients’ prescription benefit information.
  • Appropriately communicates and directs all questions or inquiries from patient or families that are outside the scope of taking the medication history to the appropriate caregivers or staff.
  • Provides high quality customer service to patients, family members, and care team members.
  • Documents all appropriate findings excellently and consistently into the EHR for ease of collaboration amongst providers, support staff, and other refill clinic staff.
  • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Other related duties as required.
  • Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
