About The Position

Working both autonomously and as part of the interdisciplinary team, the PPS Coordinator is responsible for the group process that produces and accurate IRF-PAI. Gathers the treatment team’s scores, validates them with documentation in the official medical record.

Requirements

Associate degree required, Bachelor’s degree in healthcare related field preferred.
Two (2) years’ experience in an inpatient rehab setting preferred or a minimum of two (2) years in the field of rehab case management preferred.
State licensure for Registered nurse.
Basic Life Support (BLS) Certification required.
Advanced Cardiac Life Support (ACLS) Certification required.

Nice To Haves

Experience with FIMS and IRF-PPS highly preferred.

Responsibilities

Observe professional ethics in maintaining confidential information acquired regarding person, financial, medical or employment of patients, families, and employees of SARH.
Must follow and enforce accepted safety practices for patients and SARH.
Understanding of CMS rules and regulations surrounding IRF PAI Requirements including:
Proper completion and transmission of IRF PAI
Time frame requirements for completion and submission.
Penalties for missing submission deadline
Trains staff related to the necessary documentation of data and appropriate forms related to PPS
Coordinates all areas of the Functional Independent Measure (FIM) score capture and supportive documentation for the completion of the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI). Transmit completed IRF-PAI.
Provides initial and ongoing FIM and PPS education including new employee orientation and focused education as needed.
Reviews medical records to assure that clinical documentation supports FIM scores. Confers with the clinical team members regarding the scoring of FIM items to assist in resolution of any inconsistencies.
Is fair, objective, tactful, and professional in dealing with all persons.
Exhibits professional skills in conducting interviews, presentations, and business meetings.
Provides IRF orientation as deemed necessary to civic leaders, clients, and volunteers.
Reviews medical records and confers with medical staff members to assure that co-morbidities are properly identified and documented in the medical record.
Confers with the patient and clinical team members regarding the scoring of FIM items, follows up on any inconsistencies, when necessary, and assesses the patient to confirm the correct score.
Keeps administration, medical directors, and staff informed of pertinent information and occurrences.
Performs other duties as assigned within the realm of competencies.
Complies with all requirements of JCAHO, CARF, State Department of Licensing and Regulation, and other regulatory licensing agencies as applicable.
Coaches subordinates appropriately and listens to suggestions/problems.
Demonstrates effective communication with Medical Staff and other departments.
Active participation in staff recruitment and retention.
Maintains cost conscious attitude to time usage, material, and supplies.
Reviews all Rehabilitation Impairment Categories and admission ICD-10 codes to assure congruence and proper coding for the IRF-PAI. Makes any necessary changes in collaboration with the clinical team, physician, medical records department, and coder.
Works directly with any clinical department to focus on documentation trends and corrective action to assure that the Medical Record documentation supports the IRF-PAI documentation.
Continuously assess FIM scoring and FIM documentation competency and provides focused FIM training as necessary.
Participates in PPS and FIM related committees and meetings as assigned.
According to need, provides coverage at other sites for PPS coordinators.
Performs additional tasks as assigned.
