Medical Social Worker Per Diem
About The Position
The Medical Social Worker provides brief therapy regarding improved management adjustment to illness, and compliance with medical regime. Adjustment to facilitate coping, to assist with social and emotional illness associated with illness and to strengthen the family support system to improve patient's response to treatment. The Hospice MSW is a core member of the Hospice Interdisciplinary Team. Requirements
- Master's degree in social work required.
- Current license to perform as a medical social worker in the State of Connecticut required for Hospice program only.
- Valid Connecticut Driver's License and proof of current insurance coverage required.
- Minimum of three years post-graduate experience in social work with one year in a health-care related setting preferred.
Responsibilities
- Makes initial assessment of terminal and multi-problem cases. Consults on cases with agency staff via case conferences or individually, including nursing, therapeutic, social services, homemaker-home health aides. Also consults with other agencies in appropriate cases by participating in interagency conferences and attending case conferences, including discharging planning.
- Evaluates situation of the patient/family and develops social work plan of care. Provides ongoing social work services to assist patient/family with personal and environmental difficulties while referring patient/family to appropriate community resources when needed.
- Prepares clinical/progress notes and discharge summaries in accordance with licensure requirements.
