What You’ll Do: Assesses and identify the client’s holistic health care needs/concerns as part of a family and community. Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care. Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to organization protocols. Implement patient care plan activities based on treatment goals towards independent living for the patient/family. Serve as resource for staff education & training, participate in case conferences as needed. Support Home Health
