About The Position

Hospice of South Texas (HOST) is seeking a compassionate Medical Social Worker to support patients and families through end-of-life care. This role is open to LBSW, LMSW or LCSW candidates with at least one year of healthcare experience. Employer-provided CEUs available. Requirements

• LBSW, LMSW or LCSW (Texas)
• Minimum of one (1) year of experience in a healthcare setting (regulatory requirement)
• Strong communication and interpersonal skills
• Ability to manage emotionally complex situations with professionalism

Nice To Haves

• Experience in hospice, home health, hospital, SNF, dialysis, oncology, or behavioral health settings

Responsibilities

• Complete psychosocial assessments for hospice patients and families
• Provide emotional support, counseling, and resource coordination
• Participate in interdisciplinary team meetings
• Assist with care planning, coping strategies, and end-of-life support
• Maintain timely and accurate clinical documentation
• Collaborate with nursing, chaplaincy, and bereavement teams
