About The Position

The Healthcare Social Worker is an important part of the COFMC team. The Social Worker provides COFMC clients with culturally competent and ethical case management services and psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include, but are not limited to, advising family care givers, providing patient education, emotional support, and client advocacy, and making referrals for other services.

Requirements

  • Bachelor's Degree from an accredited university in Social Work or related human services field.
  • Knowledge of: Current social and economic problems.
  • Concepts and theories associated with life-span development, grief, loss and bereavement, and depression, anxiety, and other mental health conditions including addiction.
  • Basic medical terminology.
  • Common health conditions; and
  • Behavioral change strategies.
  • Ability to demonstrate knowledge base as indicated by the COFMC Competency Assessment for Behavioral Health staff.
  • Knowledge of emergency procedures, clinical policies and procedures, and infection control procedures.
  • Concise and clear writing skills.
  • Ability to follow chain of authority.
  • Ability to work cooperatively with all team members and departments.
  • Ability to work independently, requesting assistance from supervisor only as needed.
  • Ability to complete tasks effectively, accurately, and on time.
  • Ability to arrive on time and work through designated work hours.
  • Good time management skills.
  • Good communication skills.
  • Above average computer skills.
  • Valid Driver's License.

Responsibilities

  • Under the supervision of the BH Director, conduct clinical case management services, screenings for psychosocial issues, and risk assessments to COFMC clients, as identified or referred by COFMC staff.
  • Respond to crisis situations as requested by COFMC staff and/or as assigned by the Behavioral Health Director.
  • Provide internal and external referrals to clients in need of additional services.
  • Assist in the development of working treatment plans and identifying barriers to treatment compliance.
  • In collaboration with BH staff, develops and conducts support groups as needed.
  • In the absence of or in the event of staff shortage, provide assistance to the Day Treatment program.
  • Document services provided through the use of an electronic health record (EHR) in accordance to the acting practice of the Behavioral Health department in a timely and efficient manner;
  • Attend all meetings, trainings, and/or other events as deemed mandatory by the Director of Behavioral Health, and/or by COFMC Admin; and
  • Complete all other assignments and/or tasks as assigned by the Behavioral Health Director.
