About The Position

The Medical Social Worker is responsible for providing social work services to patients in their homes per physician's plan of care. DIMENSIONS: Visits per physician's plan of care. FIDRNOCT

Requirements

• Master's degree
• Masters degree is preferred
• Area of study or major: Social WorkMSW
• Current Ohio License in Social Work
• CPR Certification
• 1-3 years of job related experience3 - 5 years of job related experience
• One year experience in a health care setting (hospital, clinic rehabilitation center, home care, etc.3-5 years of experience providing clinical services; experience in home care or community/outpatient setting preferred.
• Computer skills preferred
• Must be capable of standing, stooping, bending and lifting up to 50lbs.
