We are currently seeking a Medical Social Worker (MSW) to join our San Jose , CA team. Requirements
- Master's degree in Social Work (MSW) – Required
- 2+ years of MSW experience in a Home Health or similar healthcare setting – Required
- Strong interpersonal, organizational, and counseling skills
- Valid Driver's License and reliable transportation
Responsibilities
- Assess the psychosocial needs of patients and families to develop individualized care plans.
- Provide counseling and emotional support to patients and caregivers.
- Assist in connecting patients and families to community resources, financial assistance, and legal aid.
- Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning.
- Document all services provided in compliance with agency policies and regulatory requirements.
