Requirements

• Accredited master’s degree in social work
• Minimum one year of healthcare experience
• Current CPR Certification
• Licensed driver with an insured automobile

Nice To Haves

• Home health or hospice experience preferred

Responsibilities

• Assess the psychosocial status of patients
• Provide community and governmental resources
• Conduct social evaluations and plan interventions
• Develop individualized care plans
• Assist physicians with social and emotional factors
• Participate in quality assessment performance improvement
• Liaise between patients and community agencies
• Maintain and develop contracts with public and private agencies
• Assist with bereavement coordination
