Requirements

Master's Degree in Social Work is required
Must possess a valid driver's license

Nice To Haves

At least 1 year of social work experience in the healthcare field preferred

Responsibilities

Assessment of psychological, emotional and/or economic factors affecting patient's limitations and potential for a lack of improvement
Collaborates with the physician and other members of the health team to develop a Plan of Care
Comply with all aspects of the company's compliance and HIPPA regulations
