About The Position

We’re looking for a PRN Home Health and Hospice Social Worker to provide vital emotional, social, and resource support to patients and families as part of our collaborative care team. Schedule: Flexible PRN Schedule Areas of Coverage: Roanoke and Surrounding Areas
Requirements
- Master’s Degree in Social Work (MSW) from an accredited program preferred
- At least one year of experience in a healthcare setting; two years preferred
- CPR certification and valid driver’s license required
- Criminal background check required
Nice To Haves
- Certification from the Academy of Certified Social Workers (preferred or in progress)
Responsibilities
- Complete psychosocial assessments and participate in patient admissions
- Develop and implement individualized care plans
- Provide short-term counseling, crisis intervention, and discharge planning
- Help patients access community resources and navigate challenges
- Document visits and collaborate with physicians and clinical staff
- Supervise Social Work Assistant monthly
- Participate in team meetings, in-services, and ongoing education
