Requirements

  • Master’s degree in Social Work (MSW)
  • Minimum 1 year of healthcare experience ( hospice experience preferred )
  • Knowledge of hospice philosophy and end-of-life care
  • Strong communication and organizational skills
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle

Responsibilities

  • Conduct psychosocial assessments for patients and families
  • Develop care plans and provide counseling for stress and coping challenges
  • Offer referrals for practical and environmental needs
  • Serve as liaison between patients, families, and community agencies
  • Support the care team in understanding social and emotional factors related to health and end-of-life issues
