About The Position
As a Hospice Social Worker, you will provide emotional support, counseling, and resource coordination for patients and families facing end-of-life care. You’ll collaborate with an interdisciplinary team to ensure holistic care.
Requirements
- Master’s degree in Social Work (MSW)
- Minimum 1 year of healthcare experience (hospice experience preferred)
- Knowledge of hospice philosophy and end-of-life care
- Strong communication and organizational skills
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
Responsibilities
- Conduct psychosocial assessments for patients and families
- Develop care plans and provide counseling for stress and coping challenges
- Offer referrals for practical and environmental needs
- Serve as liaison between patients, families, and community agencies
- Support the care team in understanding social and emotional factors related to health and end-of-life issues
