As we continue to grow, we are seeking an experienced and compassionate Medical Social Worker (MSW ) to join our interdisciplinary hospice care team. Position Details Job Title: Medical Social Worker – Hospice Pay Range: $40.00 – $50.00 per hour Status: Part‑Time Service Areas: Santa Clara County, Alameda County & San Mateo County Job Summary The Medical Social Worker provides psychosocial assessments, emotional support, counseling, and resource referrals for hospice patients and their families. You will collaborate closely with the interdisciplinary team to ensure holistic, patient‑centered care aligned with each patient’s hospice plan of care.

Requirements

  • Master’s degree in Social Work (MSW/DSW/PhD) from a CSWE‑accredited school.
  • 1+ year of social work experience in a healthcare setting (hospice experience preferred).
  • Current CPR certification.
  • Valid driver’s license, reliable insured vehicle, and ability to travel within assigned service areas.

Responsibilities

  • Conduct comprehensive social work evaluations and develop individualized care plans.
  • Provide emotional support, counseling, and crisis intervention to patients, caregivers, and families.
  • Deliver services ordered by the physician and outlined in the hospice plan of care.
  • Communicate regularly with physicians, RNs, and interdisciplinary team members.
  • Assist the care team in understanding key social, emotional, and environmental factors affecting patient health.
  • Prepare timely and accurate clinical documentation for all patient encounters.
  • Offer community resource referrals and practical assistance to patients and caregivers.
  • Serve as a liaison between families and community agencies, ensuring seamless coordination of services.
  • Maintain positive, collaborative relationships with internal staff and leadership.
  • Participate in case conferences, discharge planning, and development of the total plan of care.
  • Maintain and develop partnerships with public and private agencies to support patient needs.
