Requirements

  • Master’s degree in social work and licensed in the state of Louisiana
  • Hospice social work experience required
  • CPR certification, valid driver's license, active auto insurance, and reliable transportation
  • Knowledge of state and federal home health regulations
  • Strong interpersonal skills; good communicator, empathetic, compassionate, and resourceful

Responsibilities

  • Provide home-based social services to patients dealing with end-of-life issues
  • Work as part of a hospice team, which will include an RN, LPN, CNA, Spiritual Counselor and Hospice Aides focused on the patient’s plan of care and individual goals
  • Monitor a patient’s psychosocial condition and identify social and emotional needs
  • Conduct patient assessments and document progress
  • Provide counseling, community resource planning, crisis intervention, and advocacy
  • Consult with family and caregivers on the patient’s plan of care
