Service Areas: Southern NH Greater Nashua and Manchester communities Schedule: Once trained and working independently, 24 hrs. will be set schedule; includes working one Saturday or Sunday approx. every 3 months and 1 holiday a year or every other year. Requirements

  • Master of Social Work (MSW) from a school of social work accredited by the Council on Social Work Education; or Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work; and
  • Has 1 year of social work experience in a healthcare setting.
  • Satisfactory background screening.
  • Satisfactory motor vehicle record.

Nice To Haves

  • Master’s degree in Social Work for Hospice Division
  • Licensed by the State of New Hampshire
  • Homecare experience
  • Hospice or end of life experience

Responsibilities

  • Conducts psychosocial assessments at the start of patient service.
  • Works with the patient/family to develop a plan of care with specific interventions to support progress on the medical plan of care.
  • Provides linkage to community services that will support the medical plan of care.
  • Works as a member of the clinical team to provide a better understanding of patient’s psychosocial and emotional needs as related to the care plan.
  • Works collaboratively with clinical staff and physician’s office toward a coordinated discharge plan.
  • Schedules patient visits according to patient needs and physician orders.
  • Assists patients in applying for needed funds or other benefits they are eligible for.
  • Documents in patient electronic records by assigned deadlines.
  • Attends staff meetings, case conferences, mandatory in-services and community activities.
  • Provides excellent customer service.
  • Performs other duties and completes projects that are assigned.
