About The Position

The part-time Community Medical Respite Worker plays a critical role in supporting individuals experiencing homelessness who are recovering from acute medical conditions and require a safe, supportive environment to heal. This position involves providing compassionate care, coordinating with healthcare providers, and assisting clients in accessing necessary medical and social services. This position also prepares meals, assists with medication administration, and coordinates transportation (as needed). The worker ensures that clients' basic needs are met while promoting their physical and emotional well-being during their respite stay. By fostering a respectful and trusting environment, the role helps reduce hospital readmissions and supports clients' transitions to stable housing and ongoing care. Ultimately, the Community Medical Respite Worker contributes to improving health outcomes and quality of life for vulnerable populations within the community.

Requirements

  • High school diploma or equivalent.
  • Experience working with vulnerable populations, preferably in healthcare, social services, or community outreach settings.
  • Basic knowledge of medical terminology and comfort working in a healthcare-related environment.
  • Strong communication and interpersonal skills to effectively engage with clients and multidisciplinary teams.
  • Ability to maintain confidentiality and adhere to ethical standards in client care.

  • 32-hour Medication Aide training and certificate
  • Experience working specifically with homeless or medically vulnerable populations.
  • Training in trauma-informed care, mental health first aid, or substance use disorder support.
  • Familiarity with community resources related to housing, healthcare, and social services.
  • Bilingual abilities or cultural competency relevant to the community served.

Responsibilities

  • Provide direct support and care to clients recovering from illness or injury in a community-based medical respite setting.
  • Monitor clients’ health status and communicate changes or concerns promptly to medical staff and case managers.
  • Assist clients with activities of daily living, including medication management, hygiene, nutrition, and mobility as needed.
  • Coordinate with healthcare providers, social workers, and community resources to facilitate comprehensive care plans.
  • Maintain accurate documentation of client interactions, care provided, and progress toward recovery goals.
  • Create a safe, clean, and welcoming environment that respects client dignity and privacy.
  • Support clients in accessing housing resources, mental health services, and other social supports to promote long-term stability.
  • Participate in team meetings and contribute to continuous quality improvement initiatives within the respite program.
