About The Position

The Medical Records Coordinator is responsible for managing and maintaining participant medical records in compliance with PACE, CMS, HIPAA, and organizational requirements. This role supports the timely collection, organization, release, and maintenance of medical information for both prospective and active participants and serves as a supporting member of the St. Paul’s PACE Interdisciplinary Team (IDT). Key functions include requesting and releasing medical information, preparing and maintaining electronic participant records, ensuring records are complete and audit‑ready, and supporting quality assurance and compliance activities. The Medical Records Coordinator works closely with Center leadership and the IDT to promote continuity of care through accurate and timely medical record management. This position falls under the Care Coordination Department and is under the direct supervision of the Care Coordination Supervisor, while providing day‑to‑day administrative and operational support to Center Directors, Supervisors, and the Interdisciplinary Team.

Requirements

  • High school diploma or equivalent (required)
  • One year experience in a medical records or healthcare administrative role

Responsibilities

  • Request, receive, track, and release medical records for prospective and active PACE participants
  • Prepare, organize, and maintain participant electronic medical records within the EHR system
  • Scan, index, attach, and reconcile internal and external medical documentation in the EHR
  • Ensure medical records are complete, accurate, and maintained in accordance with PACE and CMS requirements
  • Manage medical record requests and release of information in compliance with HIPAA policies
  • File, copy, fax, and electronically distribute medical information as needed
  • Support quality assurance, compliance, and audit activities, including chart reviews and documentation completeness checks
  • Assist Center Directors and Supervisors with monitoring documentation timelines and record readiness
  • Communicate and collaborate with IDT members, providers, vendors, and external partners
  • Support onboarding of new participants through timely chart preparation and record coordination
  • Perform other related duties as assigned to support Care Coordination and program operations
