Requirements

  • RN, LPN, BA, BS in human services field required.
  • Experience with medical or death investigations, oversight of medical records or care preferred.
  • High school diploma or equivalent required.
  • 3 years' experience in the field required.
  • Prior experience conducting investigations preferred.
  • Excellent organizational skills and interpersonal skills required.
  • Prior experience training required.
  • Computer proficiency in Word and Access preferred.
  • Qualified NYS Driver's License required.

Responsibilities

  • Conducts thorough investigations of reported incidents and allegations of abuse, with a medical focus.
  • Interviews staff, analyzes collected information, identifies systemic issues, and develops detailed investigation reports with recommended corrective actions.
  • Completes administrative death investigations, including record reviews and solicitation of documents from outside providers (hospital, hospice, rehabilitation, etc.).
  • Ensures proper oversight of the incident reporting process.
  • Monitors entries in the Incident Reporting Management Application (IRMA), ensuring that all data is complete, accurate, and updated through to the closure of each case.
  • Maintains organized and confidential incident files in accordance with regulatory and agency standards.
  • Ensures that all incident notifications and documentation submissions to oversight agencies are completed within required timeframes.
  • Serves as a point of contact with oversight agencies, providing requested documentation and updates related to investigations as needed.
  • Serves as a member of the Human Rights Committee and provides feedback on requests made through the Human Rights designated mailbox.
  • Maintains and reviews information in the Human Rights database to ensure consents remain current and relevant.
  • Develops and delivers both general and targeted training to agency staff on topics including incident reporting protocols, abuse prevention, and regulatory compliance.
  • Performs additional responsibilities and tasks assigned by the supervisor, in support of departmental and agency goals.
