Requirements

  • The Credentials: A Master’s or Doctorate from an accredited program (NP or PA) with a current state license, Board Certification, and an active DEA.
  • The Mindset: You are independent yet collaborative, a master of time management, and a compassionate communicator who treats every patient with profound respect.

Nice To Haves

  • The Experience: A background in Geriatrics, Post-Acute, or SNF settings is strongly preferred. You understand the unique rhythm of the post-acute environment.

Responsibilities

  • Be the Clinical Lead: Perform comprehensive assessments and manage acute and chronic conditions in complex, medically fragile populations.
