POSITION SUMMARY: In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services .

Requirements

• Qualified MD/DO
• Active , unrestricted, Medical Licenses in Illinois and at least 10 other state licenses.
• Excellent clinical jud gment and decision-making skills.
• Must have reliable transportation to travel to assigned buildings.
• Commitment to providing patient centered care and improving health outcomes.
• Familiarity with electronic health record ( EHR ) systems and healthcare technology.
• Ability to travel within 1- 2 hours of assigned region/home residence

Responsibilities

• Responsible for establishing the Tapestry Health patient relationship for our C hronic Care Management and Initiating Patient Visit services .
• Be available at a minimum of 10 weekdays per month to conduct approximately 3 75 clinical visits, both onsite and remotely.
• Distance will be roughly 1 hour from home residence and we will pay mileage.
• Document patient encounters accurately and thoroughly within our platform at time of visit .
• Provide end of day report to program administrators to track progress for completion of visits.
