About The Position

The medical coordinator provides clerical support to the nursing staff and direct support to the people served at the residential site. Promotes Person-Centered planning for people receiving services and their family members/guardians. Collaborates with the entire residential team including DSPs, clinicians, community agencies, and others involved in medical/nursing care as appropriate. Provide oversight and guidance to direct care staff of designated shifts. Ensure completion of required duties for each shift.

Requirements

  • High school diploma or GED
  • 1 year of related experience preferred
  • 18 years of age
  • Valid Driver’s License that meets agency policy
  • Medical competency-based training
  • Excellent interpersonal skills
  • Ability to handle multiple tasks simultaneously
  • Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling and climbing
  • Lifting requirement of 35 pounds
  • Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan
  • Ability to move, reposition, and place individuals in pieces of equipment
  • Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines
  • CPR certification required
  • Compliance with medical training as outline by policy

Responsibilities

  • Coordinates referrals to community based medical specialty providers
  • Assists with scheduling and attending appointments and procedures with persons served as needed
  • Coordinates incoming and outgoing provider correspondence
  • Assists in delegated nursing care as directed by RN
  • Ensures there is sufficient stock of medications, and the medication room is organized and clean
  • Communicates with clinical staff to ensure follow up to medical appointments in their clinical area.
  • Liaison with the pharmacy.
  • Checks MARS for errors and completes necessary paperwork including use of PRN medication and ensuring documentation is complete.
  • Communicates with person served, families, and nurse regarding medical appointments and outcomes.
  • Works closely with PT/OT to understand therapy concerns and adaptive equipment needs.
  • Updates medical information in ready to go packets and individual files as needed.
  • Makes notifications to day programs related to individual health needs and concerns.
  • Attends support team/clinical meetings
  • Maintains a safe orderly environment conducive to the health, safety and wellbeing of individuals and employees. Employing infection control practices
  • Maintains Medication Administration Certification and administers medications
  • Participates in educational and training opportunities
  • Supervises and schedules the tasks/activities to ensure the services and care to each person ( i.e. shift responsibility, medical care). Provide supervision and training of direct care staff on designated shifts.
  • Assist the Senior Residential Supervisor with staff development, coaching, and feedback on performance and corrective action as required.
  • Is responsible for the adherence to the Medical Coordinator manual
  • Ensures efficient, confidential, and comprehensive record keeping as mandated for individual and staff related activities/incidents and site maintenance.
  • Safely transports individuals to and from activities and appointments.
  • Complies with all agency policies and procedures.
  • Other duties as assigned.
