About The Position

The Medical Coordinator is responsible for arranging and coordinating appropriate health services for children in care, including urgent, emergency, and preventive care. This role ensures children receive timely and comprehensive medical care while maintaining complete and confidential medical records in accordance with agency policy, program requirements, and applicable licensing regulations. The Medical Coordinator ensures effective communication of children’s medical needs to case managers, clinicians, and program staff; schedules healthcare appointments; coordinates transportation to medical appointments; ensures qualified interpretation and translation services are available when needed; and serves as a liaison to community healthcare providers. This position supports the organization’s commitment to trauma-informed, culturally responsive, and high-quality care for all children served.

Requirements

  • Must be at least 21 years of age
  • Valid Driver License
  • Ability to pass required background clearances
  • Licensed Practical/Vocational Nurse (LPN/LVN), Registered Nurse (RN), or Bachelor’s degree in a relevant health-related field with at least one year of experience in healthcare coordination, child welfare medical services, or public health programs.
  • Experience working with children, families, or vulnerable populations preferred.
  • Bilingual in Spanish is requested but not required.
  • Read, analyze, and interpret business, professional, technical, or governmental documents.
  • Write reports, business correspondence, and procedure manuals.
  • Effectively present information and respond to questions from managers, customers, and the public.
  • Comprehend and apply principles of advanced calculus, algebra, and advanced statistical theory.
  • Work with concepts such as limits, rings, quadratic and differential equations, and proofs of theorems.
  • Apply logical thinking to a wide range of intellectual and practical problems.
  • Dealing with nonverbal symbolism (e.g., formulas and equations) in difficult phases.
  • Deal with a variety of abstract and concrete variables.
  • Proficiency in MS Office (Word, Excel, Outlook)
  • Proficient in the use of standard office equipment.
  • Electronic Medical Record and UC Portal Software

Responsibilities

  • Arrange and coordinate appropriate health services for children in care, including urgent, emergent, and preventative medical services.
  • Maintain complete, accurate, and confidential medical records in accordance with agency policy, program standards, licensing regulations, and funding requirements.
  • Ensure appropriate communication of medical and health needs to case managers, clinicians, program leadership, and other relevant staff.
  • Schedule and coordinate medical, dental, vision, behavioral health, and specialty healthcare appointments.
  • Coordinate transportation to medical appointments in accordance with agency transportation and safety policies.
  • Ensure qualified interpretation and translation services are available for medical appointments and healthcare communications when needed.
  • Serve as liaison between the program and community healthcare providers, hospitals, specialty clinics, and public health entities.
  • Monitor medical compliance requirements, including immunizations, physicals, medication management, and follow-up care.
  • Prepare, compile, submit, and maintain medical documentation, reports, and records required for programming, licensing, and internal compliance monitoring.
  • Review programs portals and internal systems to ensure required medical documentation is uploaded accurately and promptly.
  • Provide medical status updates to authorized sponsors, family members, and staff as appropriate and in accordance with confidentiality regulations.
  • Participate in multidisciplinary team meetings and service planning to ensure medical needs are incorporated into individualized service plans.
  • Support discharge planning by ensuring continuity of medical care and transfer of appropriate medical documentation.
  • Adhere to mandated reporting laws and child abuse reporting requirements.
  • Attend required trainings, meetings, and professional development activities.
  • Perform other duties as assigned by supervisor or leadership.
