About The Position

Reporting to the Manager of HIV Services, the Medical Case Manager (MCM) provides medical and social services case management to adult clients with HIV/AIDS in the Codman community. This role provides high-level advocacy and for patient’s clinical, psychosocial and social determinant of health needs (SDoH). The MCM provides comprehensive case management services, manages biannual referrals for state and Federal medication assistance programs, as well as other referrals for short-term health navigation. MCMs work in tandem with a robust team of care providers, including physicians, nurses, social workers and medical assistants, to provide optimum care for patients and their ongoing and emerging health needs.

Requirements

  • Associates degree or minimum of 2-years progressive professional experience
  • Knowledge and experience with HIV services, including HIV education/prevention, outreach, counseling, emotional/psychological support and or care management
  • Awareness of substance abuse issues, including detox, rehabilitation and recovery
  • Ability to work within a wide community and social service network
  • Proven ability to work with patients from culturally, ethnically and racially diverse backgrounds
  • Familiarity with Electronic Medical Records (EMRs) and Electronic Health Records (EHR) systems; this role requires keen documentation skills
  • Ability to speak Haitian Creole highly desirable

Responsibilities

  • Provides case management services to HIV positive patients
  • Supports new patients with intake and onboarding, acclimating them to the clinical processes and providing emotional support, as needed\
  • Reinforces self-determination and utilizes patient teaching opportunities to ensure medication adherence and patient engagement
  • Collaboratively works with MD, RN and other MCMs to ensure optimal care
  • Refers clients to community agencies to support SDoH needs
  • Conducts monthly office and/or telephonic patient visits, updating plans of care and supporting positive patient outcomes
  • Maintain up-to-date monthly activity and variance reports on high-risk patients
  • Attends team meetings as needed and as requested by clinical team leaders
  • Completes all paperwork within the required time frame
