This role works with a multi-disciplinary team to care for highly vulnerable patients in our medically monitored residential substance use disorder program(also known as 3.7R level of care). The case coordinator plays a critical role, providing care coordination and ongoing support for patients who have significant medical or psychiatric conditions. The case coordinator may also provide support across medical programs including MAT, Residential, Withdrawal Management, and Psychiatric services. Specific duties include daily check-ins, scheduling outside appointments, escorting patients to appointments, and care coordination. This role works closely with a team that includes addictions counselors, medical providers, nurses and peers to provide excellent care to patients in this program. The team’s philosophy is rooted in radical kindness, harm reduction, and trauma-informed care.
