About The Position

The Medical Care Manager at AIDS Healthcare Foundation (AHF) is responsible for supporting clients within the Ryan White Care Act Program. This involves collaborating with providers and the Healthcare Center team to ensure clients receive necessary support and access to resources, including education on program eligibility. Key duties include conducting Needs Assessments, developing Action/Care Plans, and providing ongoing patient education about diagnoses and medications. The manager will re-engage clients who have dropped out of care and support new patients identified as high-risk. For contracts that include psychosocial needs, the manager will address mental health and basic needs such as housing, food, and clothing, developing relationships with relevant programs and agencies to facilitate referrals and support. Ongoing retention calls are also a part of the role to ensure continuity of care. The Medical Care Manager enhances AHF's reputation by identifying client strengths, opportunities for growth, and assisting with realistic goal setting. They coordinate care for people living with HIV (PLWH) to ensure viral load suppression and retention in medical care, embodying the safety net function of the Ryan White Care Act. This role directly contributes to AHF’s success in meeting Ryan White contract deliverables and measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services. The manager ensures coordination, follow-up, and completion of medical treatments and care plan interventions, providing timely access to health and support services and continuity of care through ongoing client and family needs assessment. They also verify client eligibility for the ACA program and assist with enrollment. The position requires a passionate advocate for AHF's initiatives, adherence to AHF Core Values and Mission, and adaptability in a dynamic environment. The Medical Care Manager works collaboratively to improve healthcare outcomes, support medication adherence, address barriers to care, and provide brief interventions focused on long-term goals, developing and monitoring individual service plans with clients and providers.

Requirements

  • Minimum Bachelor's level degree in a social science or health discipline
  • Minimum of 3 years of Case Management experience
  • Knowledge of community resources
  • Competence in Cultural and linguistic Diversity

  • Knowledge of Ryan White Services

Responsibilities

  • Utilize experience, skills, and education to thoroughly understand the Ryan White Care Act Program within their specific area.
  • Work with providers and Healthcare Center team so that every Ryan White client receives the support they require and access to available resources.
  • Provide education and assistance so that eligibility for the program is understood.
  • Meet with clients and complete Needs Assessments, develop Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications.
  • Reengage clients who dropped out of care or provide support to new patients determined to be at high risk of dropping out based on risk factor assessments, missed appointment data, and service utilization data.
  • Understand mental health needs and basic needs such as housing, food, and clothing for Ryan White contracts that combine psychosocial needs within Medical Care Management.
  • Develop relationships with programs and agencies serving mental health and basic needs and facilitate referrals, support, and assistance for patients who lack those services.
  • Conduct ongoing retention calls to ensure no gap in retention efforts and to uphold radical cooperation across divisions.
  • Identify strengths and opportunities for growth, and assist clients with realistic goal setting.
  • Coordinate care for people living with HIV (PLWH) eligibility and ensure they achieve and maintain viral load (VL) suppression and retain medical care.
  • Assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions.
  • Ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems.
  • Verify the client’s eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.

  • Work as a part of a close-knit team to improve health care outcomes, support clients taking all their medications as prescribed, and address barriers to care and treatment.
  • Provide brief interventions focused on a long-term achievable goal.
  • Collaborate with providers to mutually develop and establish an action or individual service plan with each client that identifies patient needs, goals, and planned course of action.
  • Monitor and document the changing needs of the client through personal contact and by reviewing the care plan at least every six months.
