About The Position

This position is a HYBRID role, requiring 1-day onsite work at Mission Viejo, CA. Care Management is a collaborative practice model that includes patients, nurses, social workers, physicians, other practitioners, caregivers, and the community. The care management process emphasizes excellent verbal and written communication and facilitates care along a continuum through effective resource coordination. The primary goal of the Care Manager is to advocate for and assist the patient in achieving optimal health, accessing care, and appropriately utilizing resources. The Care Manager utilizes assessment, planning, implementation, coordination, monitoring, and evaluation of the plan of care to meet the patient's individual healthcare needs. Requirements

• Graduate of an accredited school of nursing.
• California Registered Nurse License upon hire.
• 3 years of Clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.

Nice To Haves

• Bachelor's degree in nursing or master's degree in Related field.
• Certification in Case Management (CCM, ACM) upon hire.
• Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care upon hire.
• 3 years of experience in utilization management or case management.

Responsibilities

• Facilitate care along a continuum through effective resource coordination.
