About The Position

An Ending HIV Epidemic RN Medical Care Manager contributes to AHF by utilizing intensive nursing clinical skills and understanding the Ryan White and Ending HIV Epidemic Programs within their specific area. With a caring and knowledgeable focus, you will work collaboratively with the patients, our physician, other providers, the Care Management team, and the Healthcare Center team to address the needs identified in the patient’s health and psychosocial risk assessment and implement a plan of care based upon the needs identified. You will also need to facilitate and identify service options for addressing a client’s needs. The EHE RNMCM will consult with clinicians to develop treatment regimens that ensure optimal clinical outcomes, address treatment adherence education needs, provide coaching to promote adherence and monitor the results of referrals to complete Comprehensive Health Assessments, develop a Coordinated Plan of Care, and provide ongoing education for clients to self-manage their HIV by keeping their healthcare appointments and taking their prescribed medications. You will also conduct ongoing retention calls to ensure no gap in retention efforts and to uphold radical cooperation across divisions. A EHE RN Medical Care Manager enhances the reputation of AHF by utilizing strong core nursing principles and meeting our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your clinical skills, you will bring the essence of the Ryan White HIV/AIDS Ending the HIV Epidemic (EHE) Initiative - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. By assessing client needs and developing a coordinated POC, strengthening the HIV Care Continuum by expanding linkage and treatment services using multidisciplinary Intensive Care Teams (ICTs), your work will directly contribute to AHF’s success in meeting the deliverables of our EHE contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures of the Health Resources and Services Administration (HRSA). You will assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessments of the client’s and other key family members’ needs and personal support systems. You will verify the client’s eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services. You will be a passionate advocate for our top initiatives. To be successful as a EHE RN Medical Care Manager it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. The EHE RN Medical Care Manager will work as a part of a close-knit team to provide intensive, coordinated healthcare interventions to assist patients to self-manage their HIV. Coordinate care for persons living with HIV (PLWH) eligible for EHE Initiative services, ensuring they achieve and maintain viral load (VL) suppression and are retained in medical care. Communicate with healthcare providers to coordinate care for comorbid chronic or disease conditions. You will monitor and document the client's changing needs through personal contact and by reviewing the care plan at least every six months.

Requirements

  • Bachelor’s degree in Nursing (BSN), or License Practical Nurse (LPN) with a minimum of 1 year of Case Management experience.

Nice To Haves

  • Knowledge of Ryan White Services is preferable.
