Requirements

  • Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field.
  • Minimum of 3 years of experience in a healthcare or medical setting, preferably in care coordination or case management.
  • Proven track record of working with diverse patient populations and managing patient care plans.
  • Experience in collaborating with healthcare professionals to ensure comprehensive patient care.
  • Strong understanding of healthcare systems, medical terminology, and patient care protocols.
  • Excellent communication skills, both verbal and written, with the ability to effectively interact with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team, demonstrating strong organizational and time management skills.
  • Proficient in using electronic health records (EHR) systems and other healthcare-related software.
  • Demonstrated ability to handle sensitive information with discretion and confidentiality.
  • Problem-solving skills with the ability to assess patient needs and develop appropriate care plans.

Responsibilities

  • Coordinate patient care activities and ensure seamless communication between healthcare providers, patients, and their families.
  • Develop and manage individualized care plans tailored to meet the specific needs of each patient.
  • Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
  • Serve as a liaison between patients and healthcare professionals to facilitate access to medical services and resources.
  • Educate patients and their families about treatment options, healthcare processes, and available support services.
  • Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and standards.
  • Collaborate with interdisciplinary teams to enhance the quality and efficiency of patient care.
  • Identify and address barriers to care, advocating for patient needs and ensuring timely access to services.
