About The Position

A team member of nursing service, that participates in the delivery of health care to the patients of Pueblo Community Health Center.

Requirements

  • A current Colorado license - Licensed Practical Nurse (L.P.N.) or Medical Assistant who graduated from an accredited program
  • Current B.L.S. with skills demonstration C.P.R. certification for Healthcare Providers required
  • One year as an L.P.N. or Medical Assistant
  • Valid driver's license and auto insurance as required by law
  • Essential to have ability to understand information/instruction, be able to exchange verbal information with others and ability to present information/instruction verbally.
  • Essential to have ability to read and understand written materials and to compose information/instruction in a written form.
  • Essential to have ability to memorize.
  • Essential to have ability to reason mathematically, have analytical skills and be able to problem solve.
  • Essential to have excellent organizational skills
  • Must have computer skills required to track patient information used in computerized tracking system; and as applicable, generate mailing lists of patient populations tracked in systems.

Nice To Haves

  • Current certification or registration through NHA, AMT, AAMA or other recognized credentialing organization is preferred.
  • Ability to speak Spanish preferred.

Responsibilities

  • Ensure the function and activities of this department to embrace the philosophy, mission, values and Communicate with Heart service model supported by the Board of Directors of Pueblo Community Health Center, Inc.
  • Adhere to the guidelines and procedures of Pueblo Community Health Center, Inc.
  • Knowledge of all case management programs (WWC, EIS, Colorectal Cancer, Geriatric, Adolescent and Pediatric Immunizations).
  • Case managers assigned to program(s) and duties within the programs per Clinical Programs Nursing Manager.
  • Knowledge of Next Gen scheduling, insurance, patient demographics, medical discharge, etc.
  • Knowledge of Next Gen EPM and EHR
  • Knowledge of CIIS and ECast
  • Knowledge of all Electronic Tracking Systems used within the Department.
  • Assist a Medical Provider in providing patient care. This will include the following, but not limited to:
  • Provides the information to the patient concerning preparations for laboratory/radiographic examinations. Also, will serve as an informational source to the patient concerning Physician Referrals, Medical Triage, and Medications prescribed by the Medical Provider.
  • Utilize appropriate forms used in patient care in medical record; (Diabetic flow sheets, WCC, Past Medical History, Health Care Maintenance Depression Screen, etc.) during EMR outage
  • Per medical provider's order: may schedule laboratory, radiographic studies using appropriate documents per Nursing P & P.
  • Administration of immunizations/medications as prescribed by the medical provider and document per nursing policy & procedure manual.
  • Adhere to all nursing and infection control policies and procedures.
  • Be a team member and participate in ongoing evaluations of systems
  • Provide nursing/medical assistant health care services appropriate to the age-specific needs of all patients in accordance with the principles of growth and development for infant, child, adolescent, adult and geriatric life cycles.
  • Maintain current licensure and CPR certification.
  • Perform other related duties as assigned by Clinical Programs Nursing Manager.
