About The Position

This position supports KCHC’s mobile medical operations and community outreach programs in the Community Outreach Department. The role provides direct patient care and clinical support under the supervision of licensed providers while also addressing broader social determinants of health. Duties include clinical tasks such as vital signs, drawing blood, medication administration, and specimen collection, as well as patient education and outreach activities like home visits, health screenings, and case management. The primary goal is to bridge healthcare and community resources to improve health outcomes, promote equity, and support the mission of KCHC.

Requirements

  • High school diploma or equivalent required
  • Certified or Registered Medical Assistant required and in good standing.
  • CPR/BLS certification (or ability to obtain within 6 months).
  • Valid NC driver’s license with acceptable driving record and eligibility for company insurance.
  • Experience in healthcare, nonprofit, or community-based organizations.
  • History of working with individuals and families from diverse cultural and socioeconomic backgrounds.
  • Strong written and verbal communication skills.
  • Ability to work collaboratively with internal departments and community partners.
  • Proficient in Microsoft Office Suite
  • Ability to safely and efficiently operate mobile unit systems and support clinical staff in patient care delivery.

  • Must successfully pass required background checks in accordance with organizational policy.
  • Must comply with all organizational credentialing and screening requirements.

Nice To Haves

  • Associate’s degree preferred.
  • Completion of an accredited Medical Assisting program preferred.
  • 1–3 years in a clinical or healthcare setting preferred.
  • Bilingual (English/Spanish) preferred

Responsibilities

  • Maintain strict confidentiality regarding patient records while utilizing a team-based approach to care.
  • Schedule/reschedule appointments, conduct patient calls, and assist with transportation as needed.
  • Accurately measure and record patients' vital signs; obtain histories and chief complaints and assist with drawing blood and other necessary screenings/tests.
  • Prepare patients for examinations and perform routine screening tests in the clinic or on the mobile health unit.
  • Accurately prepare and administer medications and injections under appropriate practitioner authorization and within scope of practice.
  • Collect and prepare specimens; sterilize and maintain instruments and exam rooms on mobile unit.
  • Support provider efficiency by performing delegated administrative tasks and EHR documentation.
  • Ensure compliance with OSHA, HIPAA, and KCHC policies.
  • Maintain a patient caseload and provide case management and navigation services.
  • Conduct outreach through home visits, health screenings, community events, and mobile health clinics.
  • Educate patients and community members about KCHC services, including sliding fee programs.
  • Assist with in developing mobile outreach schedules, ensuring there is a variety of patient payor mix and performance summaries
  • Research and connect patients to community-based resources based on individual care plans.
  • Accurately document outreach activities, charge entries, and schedules in NextGen or assigned systems.
  • Attend community meetings, internal trainings, and professional development opportunities.
