Medical Assistant - ACO Care Management
About The Position
Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Medical Practices, the largest multispecialty group in the region is looking for a full-time Medical Assistant to join our experienced ACO Care Management team in Springfield, MA. RESPONSIBILITIES: Baystate Health, ACO Care Management (CM) Medical Assistants play a vital role in helping patients and their families navigate the healthcare system, access community resources, and build healthier lives. This position is ideal for a compassionate, organized, and proactive Medical Assistant who thrives in team-based care and wants to make a meaningful impact beyond the exam room. Working as a key member of the ACO Care Management team, you will support patients transitioning from hospital to home, coordinate follow-up care, and provide education and outreach that promotes independence, wellness, and long-term health.
Requirements
- Registered or Nationally Certified MA, preferred
- Must have graduated from an accredited MA training program or equivalent training with minimum 2 years of experience in medical clinical setting
- High School diploma or GED, required
