About The Position

The Medical Assistant I Care Coordinator is primarily responsible for providing care coordination and medical scribe services as part of the Older Adult Geriatrics Program. As a secondary function, the Medical Assistant I Care Coordinator will also provide general medical assistant (MA) coverage and will carry out traditional MA clinical duties, as needed.

Requirements

  • High School Diploma or equivalent.
  • One (1) year of experience as a medical assistant in a healthcare setting.
  • Current valid certification as a Medical Assistant.
  • Excellent telephone and patient service skills.
  • Knowledge of patient billing procedures, insurance verification.
  • Ability to work efficiently and effectively.
  • Ability to work well under pressure, multi-task and handles stress well.
  • Strong analytical, employee relations, and interpersonal skills.
  • Excellent writing, business communication, editing, and proofreading skills.
  • Ability to interact effectively and in a supportive manner with persons of all backgrounds.
  • Proactive, self-motivated and able to work independently in a fast-paced environment as well as on a team with the ability to exercise sound independent judgment.
  • Ability to maintain a high level of confidentiality and a professional demeanor and must positively represent the organization at all times.

  • Must have good computer skills using Microsoft Office and the ability to use Axis departmental systems.
  • Must be able to use office equipment (i.e. copier, fax, etc.).

Nice To Haves

  • EPIC experience a plus.
  • English/Spanish bilingual preferred.

Responsibilities

  • Accurately document patient encounters in real-time during medical office and telehealth visits.
  • Record detailed patient histories, physical examinations, assessments, and treatment plans.
  • Ensure all medical documentation is completed promptly and accurately.
  • Assist providers with medical charting and electronic health record (EHR) management.
  • Schedule patient appointments, follow-up visits, and work with internal and external teams to ensure referrals and orders are completed.
  • Communicate with patients to provide education, instructions, and support regarding their care plan.
  • Assist patients with understanding and navigating their healthcare options and services.
  • Coordinate services to address any gaps in medical care such as screenings, A1c checks and immunizations.
  • Greet patients and agency visitors; direct all individuals to the appropriate location and services; be courteous, polite and helpful to the public and clients at all times when representing the organization.
  • Respect and maintain privacy and dignity of agency clients; assure client confidentiality per HIPAA regulations.
  • Assist patients in accurately completing appropriate forms, and document all information according to Axis Community Health protocols.
  • Provide clinical support services for the assigned department/provider.
  • Prepare patients for examinations according to Axis protocols and document all findings and related information according to Axis protocols.
  • Perform basic laboratory screenings according to agency protocols.
  • Perform injections under the supervision of a Medical Provider or Registered Nurse.
  • Provide clerical support for the assigned department/provider, to include preparing patient records for visits, filing laboratory and other patient reports in the medical record and maintaining all forms necessary for clinical services.
  • Clean and sterilize instruments according to protocol.
  • Complete all laboratory and autoclave control procedures according to protocol.
  • Assist all members of the clinical team as requested.
  • Maintain the assigned department and ensure that all equipment is in working order, that the area is clean and well-maintained and that sufficient supplies are available for the office and clinical operations.
  • Maintain the cleanliness of all department and patient areas before, during and after clinics.
  • Prepare the clinical area prior to the start of each clinic.
  • Participate in staff meetings, attend other meeting and training events as assigned.
  • May be required to perform other related duties, responsibilities and special projects as assigned.
