Provides pre-visit coordination, pre-and post-visit discussions, exit discharge interaction with confirmation of patient understanding, and follow-up regarding medication questions, appointments, and test scheduling, etc. Provides post-discharge patient follow-up regarding medication use and problems filling prescriptions via telephone and other electronic communications. Reviews medical records and other data sources in order to identify medication adherence barriers. Communicates this information to Clinical Pharmacy Coordinator/Pharmacotherapy Specialist or designees via oral communication, electronic and paper medical record documentation. Contacts patients that need preventive and chronic services to schedule an appointment. Documents patient outreach in the EMR and alerts providers to the needed services. Communicates with insurance providers to coordinate and obtain prior authorizations and/or medication use evaluation requirements. Collates and analyzes data. Prepare reports on monthly basis which will be used to identify areas of opportunity for continuous improvement of quality and the patient experience. Demonstrates strong communication skills and service excellence to encourage patient and family involvement in their care and treatment. Maintains documentation of interventions including productivity, outcome, management reports etc. Obtains referrals and/or authorization as required by insurance plans. Participates in all departmental activities. Demonstrate knowledge of patient safety, The Joint Commission, Department of Health standards as well as TBHC policies and procedures.
