About The Position

VCU Health at Home delivers mobile, integrated health services as part of its Continuum Integration service line to enhance the patient experience by providing comprehensive, safe and efficient in-home care as an alternative to an emergency department visit or bricks and mortar hospitalization. The Home Hospital Medic for VCU Health at Home is an interdependent member of the care team who is responsible for performing comprehensive assessments, diagnostic testing and medical treatments in a patient’s home or current residence. The practice of the Home Health Medic is focused on longitudinal assessment, participation in an existing, multidisciplinary, interprofessional treatment plan; communication with and referral to other members of the treatment team based on changing patient needs are essential functions.

Requirements

  • High School Degree or equivalent
  • Successful completion of an accredited EMT-P/Paramedic course
  • Current EMT-P/Paramedic certification/licensure in Virginia
  • BLS and ACLS Certifications
  • Valid State of Virginia Driver’s License- must have at time of hire and maintain for duration of employment a motor vehicle operator record acceptable to VCU Health’s insurance carrier/underwriter
  • Minimum of three (3) years of clinical experience
  • Demonstrated knowledge and skills necessary to provide appropriate care within the scope of practice of an AEMT, EMT-Intermediate, Paramedic – or MMAC Qualified Combat Medic/68W, Corpsman or Medical Technician/4NOX
  • Must be able to drive vehicles safely in all types of weather
  • Excellent customer service and interpersonal skills.
  • Strong problem solving, analytical and decision-making skills with ability to work independently with minimal supervision
  • Strong organizational, planning and implementation skills
  • Able to balance multiple tasks simultaneously.

Responsibilities

  • A leader in the provision of mobile healthcare using patient-centered resources in the out-of-hospital environment, the Home Health Medic examines, screens, treats, performs diagnostics, and coordinates health services for patients in the VCUMC Home Hospital program.
  • Performs under the direct supervision of the Nurse Manager to ensure quality care during daily in-person visits to patients receiving acute care at home through Home Hospital which includes daily assessments, vital sign monitoring, and patient education; lab draws, wound care, and medication administration may also be necessary and will be closely coordinated with the Home Hospital nursing and provider teams.
  • Provides quality acute/inpatient level of virtual support to the Home Hospital healthcare team if needed including triage of calls, virtual medication administration, virtual visits, telephone outreaches, education, and escalation of services both clinical and non-clinical.
  • Provides high level clinical care through the ability to perform phone and in-person triage and intervene or escalate appropriately using critical thinking skills for both inpatients and complex adult and/or geriatric patient populations.
  • Ensures acute inpatient individuals are receiving appropriate services during their Home Hospital stay including but not limited to therapy, food, consultative, mobile imaging, labs, and medical equipment to prevent un-necessary hospital readmissions.
  • Using strong communication skills, provides patient/caregiver education and ensures patients and/or caregivers are confident and comfortable in self-care management through use of in-person or virtual teach back education strategies.
  • Provides quality patient care and education in accordance with VCUMC Home Hospital policies and state regulations, as approved by the Medical Directors of Home Hospital and professional standards of care.
  • Provides accurate and legible documentation of patient care with attention to detail to accurately reflect assessment and interventions.
  • Maintains the ability to understand, utilize and efficiently integrate technology for longitudinal patient assessment, clinical interventions, physician consults and data collection related to paramedicine.
  • Performs other duties as assigned and/or participates in special projects in order to support the mission of VCUHS and department.
  • Provides assistance to team members.
  • Accepts alternate assignments, as required, graciously.
