ChristianaCare is seeking experienced and innovative Paramedics to fill the position of Mobile Integrated Health Community Paramedic. This position will work within the ChristianaCare Center for Virtual Health, Hospital at Home program. The paramedic provides in-home patient assessments, clinical and social screenings, clinical interventions, patient education, and participates in telehealth visits from the patient's home while connecting with the physician or virtual command center via Telehealth. Mobile Integrated Healthcare is the provision of healthcare using patient-centered, mobile resources in the out-of-hospital environment. At ChristianaCare, we offer Hospital at Home, which provides hospital-level care in the patient's home. Hospital at Home is designed to enhance a patient's experience by providing comprehensive, safe and efficient in-home care as an alternative to hospitalization.
