About The Position
We’re hiring a RN Nurse Assessment Coordinator (NAC)RN Nurse Assessment Coordinator (NAC) perform precise, timely resident assessments, uphold regulatory standards, and drive high-quality, person-centered care. After 90 days, this role will be added to the on-call rotation. Wage Scale Based on YOUR years of experience.
Requirements
- Current RN license South Carolina
- Strong assessment, documentation, and teamwork skills
Nice To Haves
- Long-term care or MDS experience preferred
Responsibilities
- Conduct and coordinate accurate resident assessments in accordance with regulatory requirements
- Develop, update, and monitor individualized care plans based on assessment findings
- Collaborate with interdisciplinary teams to ensure quality, person-centered care
- Train, mentor, and provide guidance to nursing staff on assessment processes
- Ensure compliance with state and federal regulations, documentation standards, and facility policies
- Review data to identify trends, opportunities for improvement, and quality initiatives
- Serve as a resource for staff and residents regarding assessment procedures and care planning
