About The Position

Priority Management is seeking an MDS Nurse to join their team at Ridgeview Rehabilitation and Skilled Nursing. The company emphasizes a commitment to providing excellent care to patients and residents, highlighting their care teams as dedicated and compassionate caregivers. The MDS Nurse is primarily responsible for the timely and accurate completion of the Resident Assessment Instrument (RAI) process and the overall care management process, from a patient's admission through discharge. This includes adhering to company policies, procedures, and Federal, State, and Certification guidelines, specifically managing the Minimum Data Set (MDS) and tracking admissions and discharges. Additionally, under the guidance of the VP of Clinical Reimbursement, the MDS Nurse may be involved in coordinating information systems operations and education within the clinical department.

Requirements

• Must possess a current, unencumbered, active license to practice as an LPN/LVN or RN in this state.
• Must have training in rehabilitative and restorative nursing practices.
• Must have a working knowledge of managed care rules, regulations, and guidelines.
• Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

Nice To Haves

• Three (3) years’ MDS experience in a nursing care facility preferred.

Responsibilities

• Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc.
• With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.
