About The Position

The Corporate MDS Nurse (LVN/RN) plays a critical role in ensuring the accuracy and compliance of Minimum Data Set (MDS) assessments across multiple healthcare facilities within the organization. This position is responsible for coordinating and overseeing the MDS process to support quality patient care, regulatory adherence, and reimbursement accuracy. The nurse will collaborate closely with interdisciplinary teams to review clinical data, identify care needs, and implement care plans based on MDS findings. By maintaining up-to-date knowledge of federal and state regulations, the Corporate MDS Nurse ensures that all documentation meets required standards and supports optimal patient outcomes. Ultimately, this role drives the continuous improvement of clinical documentation and operational efficiency across the corporate network.

Requirements

  • Current and valid Licensed Vocational Nurse (LVN) or Registered Nurse (RN) license in Texas.
  • Minimum of 2 years clinical nursing experience, in long-term care or skilled nursing facilities.
  • Demonstrated knowledge of MDS 3.0 assessment processes and CMS regulations.
  • Strong understanding of clinical documentation and resident care planning.
  • Proficiency in electronic health records (EHR) and MDS software systems.

Nice To Haves

  • Experience working in a corporate or multi-facility healthcare environment.
  • Certification in MDS coordination or related clinical documentation specialty.
  • Familiarity with quality improvement methodologies and data analysis.
  • Excellent communication and training skills for educating clinical teams.
  • Ability to travel between facilities as needed to support corporate initiatives.

Responsibilities

  • Review and validate MDS assessments for accuracy, completeness, and compliance with CMS guidelines and state regulations.
  • Collaborate with facility nursing staff, case managers, and interdisciplinary teams to ensure timely and accurate data collection and submission.
  • Provide education and training to clinical staff on MDS processes, documentation standards, and regulatory updates.
  • Monitor and analyze MDS data trends to identify areas for quality improvement and support corrective action plans.
  • Serve as a resource for clinical and administrative staff regarding MDS-related questions, regulatory requirements, and best practices.
  • Assist in preparing for audits and surveys by ensuring documentation readiness and regulatory compliance.
  • Support the development and implementation of policies and procedures related to MDS and resident assessment protocols.
