About The Position

The MDS RN or LPN is responsible for the coordination of quality care to residents, from pre-admission through discharge. The MDS RN or LPN will coordinate the written care plan and assessment and coordinate professional nursing care to residents. Why Legacy Health Services? Requirements

  • Licensed RN or LPN in the State of Ohio
  • Prior MDS 3.0 and LTC experience
  • MDS Coordinator will interact with many people so it is imperative to have constant professionalism and a pleasant disposition
  • MDS experience required

Nice To Haves

  • 1 – 2 years of experience in Long Term Care preferred
