About The Position

Orchard Post Acute, a skilled nursing facility in Fresno, is seeking a knowledgeable and detail-oriented MDS Nurse (LVN) to join our clinical team. This full-time position is ideal for a licensed vocational nurse with experience in MDS coordination, PDPM tools, and interdisciplinary care planning. Position Summary: The MDS Nurse is responsible for overseeing and coordinating all aspects of the MDS process. This includes assessing new admissions, scheduling and submitting assessments, tracking quality measures, training new coordinators, and driving care planning and compliance across the interdisciplinary team.

Requirements

  • Current LVN license in good standing with the State Board of Nursing (required)
  • Graduate of an accredited School of Nursing (Bachelor of Science in Nursing preferred)
  • 3+ years of supervisory or MDS coordination experience preferred
  • Long-term care and skilled nursing experience strongly preferred
  • Strong knowledge of PDPM tools, MDS regulations, and care planning standards
  • Proficient in Microsoft Office Suite; knowledge of PCC highly preferred
  • Strong reasoning, decision-making, and communication skills
  • Ability to maintain professionalism, accuracy, and attention to detail

Nice To Haves

Responsibilities

  • Facilitate weekly PDPM meetings and monitor PDPM tools and usage
  • Assess new admissions to determine health status and level of care
  • Track and analyze QMs and QASP performance data monthly/quarterly
  • Lead MDS and PDPM training for new MDS Coordinators
  • Coordinate interdisciplinary participation in MDS completion and care planning
  • Ensure timely, accurate MDS submissions per state and federal regulations
  • Conduct and schedule care conferences, assist with care plan development
  • Monitor and process level of care changes
  • Maintain confidentiality and HIPAA compliance across all resident records
  • Provide clinical guidance and data-driven recommendations to improve outcomes
