About The Position

The MDS Manager manages individualized care plans for Health Care Center according to State and Federal regulations; ensures care plans are followed and up-dated as needed. Monitors quality of care standards according to the CMS 5-star quality of care indicators. Essential Job Functions: Oversees the generation of a Minimum Data Set for each resident and patient and electronic transmission of required data by timeframe mandated by the state of North Carolina. Ensures resident care is delivered as ordered according to Federal and State guidelines by making rounds and reviewing residents’ charts periodically. Confirms individual Care Plan approaches are being carried out or changed as needed by interacting and communicating daily with the patient staff. Confirms weekly care plan meetings are performed on time by notifying multi-disciplinary team of care plan schedules and updates. Leads proactive change by researching health, safety, and continuing education information related to nursing and notifies DON or designee of ideas for continuous improvement of Salemtowne’s nursing care. Supports and promotes Salemtowne Shines culture and philosophy. Performs other duties as assigned.

  • MDS Knowledge: In-depth knowledgeable of medical coding ICD 10 and Medicare/Medicaid regulations.
  • Technology Savvy: Proficient in Microsoft Office 365 (Outlook, Word, Excel, Power Point, etc.).
  • Graduate of a state approved nursing program.
  • Registered Nursing (RN) license in good standing with NC Board of Nursing.
  • Two (2) to three (3) years leadership experience.
  • Two (2) years of Minimum Data Set and PDPM.
  • Must be able to lift, carry, pull, and/or push up to 50 lbs.
  • Normal sitting, pulling/pushing, kneeling, stooping, bending, reaching, talking, and hearing.
  • Standing and sitting for long periods of time.
