Santa Rosa Post Acute is looking for a detail-oriented and compassionate MDS Coordinator to join our team! Requirements
- Active LVN or RN license in California (RN preferred)
- Experience with MDS 3.0 (preferred)
- Strong understanding of Medicare/Medicaid guidelines and reimbursement
- Excellent attention to detail and organizational skills
- Ability to work collaboratively with nursing and therapy teams
Responsibilities
- Coordinate and complete MDS assessments in accordance with federal and state regulations
- Ensure accuracy of coding to support appropriate reimbursement
- Collaborate with interdisciplinary teams to develop and update care plans
- Monitor documentation and ensure compliance with regulatory standards
- Participate in care conferences and quality improvement initiatives
