At Norwood Towers Post-Acute, the MDS Coordinator is the strategic engine of our clinical operations. We are looking for a meticulous Registered Nurse who views the RAI process not just as paperwork, but as a vital tool for resident advocacy and facility excellence. You will lead our interdisciplinary team to ensure that every resident’s clinical story is told accurately, ethically, and timely. Key Impact Areas Clinical Accuracy: Lead the completion of MDS assessments and CAAs, ensuring that resident documentation reflects the high level of care provided. Regulatory Leadership: Maintain a "survey-ready" environment by ensuring total compliance with CMS and Ohio state guidelines. Collaborative Care: Facilitate interdisciplinary care plans and conferences, acting as the bridge between nursing, therapy, social services, and families. Reimbursement Integrity: Safeguard the facility’s financial health by ensuring MDS submissions accurately support Medicare/Medicaid reimbursement. Staff Mentorship: Serve as the subject matter expert, providing education to the floor staff on documentation standards and MDS requirements. Requirements

  • Active Registered Nurse (RN) license in the State of Ohio.
  • At least 1 year of dedicated MDS Coordinator experience in a Skilled Nursing (SNF) or Long-Term Care setting.
  • High proficiency with EHR systems (PointClickCare or Epic preferred).
  • An analytical approach to clinical details and the ability to navigate sensitive situations with professionalism and empathy.
